The turntables in microwaves that have them don't do an optimal job of exposing the food to the various hotspots in the microwave chamber. Adding in another gear to turn a yet smaller turntable within the main turntable would create a better rotation of food.

The smaller turntable need not be much smaller; it could take up like 80% of the main turntable's area to still give you a lot of room on which to put food.
