How annoying is it when you get an itch deep down inside your shoe, but you haven't got time to stop and take off the said item? A simple solution would be to incorporate one or two zip-type fastenings at strategic places on the footwear, allowing easy access for a finger, ballpoint pen or whatever is at hand to scratch the affected area.Simple as that.
