Just like a fantasy baseball league, really, except you get to "hire" doctors from a limited pool using a limited budget, and your ratings depend on their real world success/failure surgical record.

Annotation:

The idea of tracking surgical / hospital success and failure is still not popular, although it is an effective way (for surgeons, anyway) to improve outcomes. I like the idea of following hospitals like sports teams and surgeons like star players. Of course, privacy would prevent real details from coming out but added publicity for real results is all good.

Pretty good. It seems that you'd have to do away with the notion of seasons, though, and make it a perpetual pursuit.
